For Medical Expenses

  • Original medical receipts with diagnosis
  • Hospital bills with itemized breakdown


For Temporary Total Disablement

  • Sick leave certificate(s)
  • Letter from employer confirming your incapacity of attending to work during sick leave
  • Income proof, if necessary
Right Text: 

For Permanent Total or Partial Disablement

  • Medical certificate / report certifying the extent of injury and percentage of permanent disablement assessed
  • Submit yourself for medical examination, if required


For Death

  • Copy of the HKID or Passport of the Deceased
  • Court proclamation of presumed death in the case of disappearance
  • Prior notice of the intended date of interment or cremation
  • Findings of any post-mortem examination or inquest
  • Letters of Administration showing the identity of the personal representative(s) of the Deceased
